New Student Health Requirements

The following requirements are STATE MANDATED.



New students enrolling in an Illinois school for the first time will be required to obtain a physical examination, vision exam, and provide up-to-date immunization record regardless of grade.

If your child has been enrolled in an Illinois school, the previous school will likely send health records when requested upon enrollment. If the previous school fails to do so, it is your responsibility to get that information to the school office in a timely manner.



All Illinois children in kindergarten, Grade 2, Grade 6, and Grade 9 are required to have an oral health examination.
○ This is required for all public, private, and parochial schools.
○ Examinations must be performed by a licensed dentist, and he/she must sign the Proof of School Dental Examination form.
○ Each child is required to present proof of examination by a dentist prior to May 15 of the school year.



All children enrolling in kindergarten in a public, private, or parochial school and any student enrolling for the first time in a public, private, or parochial school are required to have an eye examination.
